CGFNS New York State
The Credential Verification Service for New York State verifies the authenticity of international education and licensure credentials for the State of New York.
The Credential Verification Service for New York State is for those health care professionals educated outside the United States who wish to practice in New York. As specified by the state, CGFNS International completes a verification of foreign education and credentials. New York State Education Department then completes the actual evaluation of the verified credentials.

Required Documents
In order to begin this verification, CGFNS must first receive a completed application from the applicant and full payment of the application fee. CGFNS does not, and cannot, begin the verification until it has received all the required materials forming a completed application. A completed application requires the completion of the following three forms:
- Authorization for Verification of Academic Records/Transcripts
- Authorization for Validation of Licensure/Registration
- Signed notarized attestation
